Required forms will be sent to you when you complete your booking. You can view the required forms at anytime by clicking the links below.
All Participants are required to complete the liability release form (waiver) and medical form:
Participants under the age of 19 years of age also must have a parent or guardian read the Risk Assessment Summary and sign an Assumption of Risk Form acknowledging that they are aware of the risks, terms and conditions.
For the 2020 Season, all participants are also required to complete a Covid Statement of Understanding. Please visit our Covid-19 Updates page for more details.
